We are seeking a talented and dedicated Social Services Associate to join our Social Services Team! Duties include, but are not limited to, working with our residents, their families and an interdisciplinary team along with ensuring quality, safe and accountable delivery of services that reflect the facilities mission, vision, philosophy and standard of care from admission to discharge. Responsible for providing services to maintain or improve residents physical, mental and psychosocial needs and well-being. Our Social Services Team are responsible for the delivery of medically related social services at our facility in accordance with current existing Federal, State and local standards as well as our established policies and procedures.

Job Requirements

• Full-time, exempt position, Monday through Friday 8 am to 4:30 pm. Weekends or extended hours as necessary.
• Ideal candidate must be a skilled communicator, detail oriented and able to organize and prioritize many tasks effectively.
• Preferred minimum of two years of experience in a long term care, hospital or other related medical facility.
• Long term care experience preferred.
• Must possess a Bachelor's Degree
